Print a QR Code

One easy way to collect daily Feedback is by printing your form’s QR code on your own materials, here are a couple of examples: 

  • Feedback cards that you give at the end of the service/purchase
  • Feedback sticker that you can print on products or in locations
  • A4 posters and stands with the QR code

QR codes are easy to set up and you can add them to your own branding and materials.


  • Go to the forms listing page
  • Edit any form
  • Click “Share” and then “Get a QR Code” and Download once your unique code shows up
  • Click “Download”

Change the QR code’s link


By default, the QR code will redirect to your form’s link. However, if you want to pass Context Attributes, you can use a QR code generator like the one available on this link

You will be able to customize the shape and brand it your way. In the URL field, add the form link and your custom fields: eg. {formlink}?product=backback&customer=123&gender=male. 

You can then generate as many QR codes as products/services you have.
